Primary Responsibilities
- Provides
clinically competent alcohol and drug treatment services; develops
clinical impressions; formulates and reviews treatment plans; plans and
conducts individual and group counseling sessions in accordance with
identified treatment goals and objectives; participates in drug screen
monitoring.
- Develops
and monitors individual treatment plans; provides follow-up contacts with
referral sources; conducts collateral contacts with family
members/significant others; provides follow-up contacts with clients to
promote client engagement in the treatment process; coordinates
information which promotes continuity of care when transferring primary
client care responsibilities to other agency employees and other
providers; identifies additional needed services and makes appropriate
referrals.
- Adheres
to standards pertaining to assessments and documentation.
Education, Certifications, and/or
Experience
- Bachelor's
Degree or comparable experience in human/social services
- At
least one year of experience in substance use disorder treatment and case
management services
- CDCA,
LPC or LSW with declared scope in chemical dependency preferred
- Meets
all standards and regulations of the Ohio Chemical Dependency
Professionals Board, the Counselor and Social Workers Board of the State
of Ohio
- Demonstrated
competencies for working with the populations served
- CPR/First
Aid Certification, De-escalation- TCN will provide upon hire
