Primary Responsibilities

?  Provides individual, group, and family case management and/or counseling under clinical supervision that meets productivity and quality requirements set by the Agency and that is within the limits of services permitted by position and scope of practice.

?  Meets or exceeds standards pertaining to services provided.

?  Accumulates the required number of billable hours of clinically allocated time based on Agency standards for position and profession

?  Meets or exceeds standards pertaining to assessments and documentation.

?  Provides only those services that have been authorized by Utilization Management and provides services within the authorized frequency and duration of services.  Requests additional services when initial authorization has been met.

?  Individualized Service Plan and Discharge Criteria with specific goals and objectives that are measurable are developed with the consumer and signed at time of the initiation of services and at time of each update. 

?  Assessment, Individualized Service Plan, Progress Notes and other documentation clearly identifies consumer's input in services by having consumer's and parent's/guardian's (if applicable) signatures when clinically appropriate

?  Completes progress notes at the end of each session and turn in within 24 hours of the service.

?  Notifies consumer and referral source (when applicable) prior to completion of the discharge summary and documents this notification on discharge within 30 days of last contact.

?  Develops and maintains quality, accurate clinical charts for all clients served.

?  Develops and maintains positive collaboration with other social service agencies serving clients.  Maintains knowledge and familiarity with referral sources within the community.  Provides consultation when needed.

Education, Certifications, and/or Experience

Associates Degree in human/social service field. Preferred: Bachelor's Degree  At least 3 years working with SMD population   At least CPR/First Aid Certifications, De-escalation- TCN will provide upon hire.  Meets ODMH licensure and certification requirements; competencies for serving the SMD population

POSITION SUMMARY

6.  Has no unexcused absences from any unit, department, and mandatory Agency meetings and training.

7.  Maintains license or registration appropriate to profession and appropriate to job requirement, including appropriate amount of continuing education at all times of employment.

8.  Maintains agency-required annual training including, but not limited to, client rights and grievances, safety training, infection control, and others as required by OHMHAS.

9.  Remains free from citations by other providers or by consumers for unethical practice and meet standard for own profession.

10.  Adheres to all organizational and departmental policies, including compliance with all behavioral and ethical expectations.

11.  Participates in at least 6 hours of supervision for every 3-month period or meet the requirements of supervision required by license/registration (whichever is greater).

12.  Other clinical duties as assigned.

MINIMUM QUALIFICATIONS

1.  Education:  Associates Degree in human/social service field, or High School Diploma

  with equivalent field experience. 

2.  Experience:  3 years working with SMD population

3.  Certification:  First Aid; CPR; De-escalation.

4.  Other:  Valid Ohio driver's license; proof of automobile liability insurance; meets

  ODMH licensure and certification requirements; competencies for serving   the SMD population.

KNOWLEDGE, SKILLS & ABILITIES

Knowledge of:    Safety rules and procedures, Agency policies and procedures, county area and resources, daily living tasks and skills, documentation requirements.

Skills:    Driving, operating basic household appliances and tools, directing and demonstrating activities of daily living, i.e. Cleaning, laundry, grocery shopping.

Abilities:    Lifting up to 25 pounds, writing legibly or use of computer,  ability to see, hear, read, and communicate information clearly, ability to recognize unusual or threatening conditions related to persons with disabilities.

WORKING CONDITIONS

Travel within the county and some travel with client present throughout surrounding counties.  May require physical demands related to assistance with activities of daily living.  Some evening and weekends may be required. Strict confidentiality is required.  May be exposed to clients who are angry/potentially violent.  Seasonal inclement weather.
